Obviously, we’d all prefer that things like that never happened, but you’re talking about a situation that’s pretty much guaranteed to bring out the worst in everyone involved. There’s on the order of a million LEOs in the US, and every one of them is an imperfect human being, just like us. Accidents will happen, no matter how much training is available. I admit that I don’t follow the issue closely, but given the size of the US population, the number of LEOs, and the general availability of firearms to the American public, the rate of these incidents does not seem to me to be excessive.
